Health check form - SMOE Renewal

I have to get another health check to renew with my SMOE school. I heard there is a form your school should give you, and then you take this form to the hospital and they do the check and charge the fee to the school. Has anyone done this? Do you have the form? My CT doesn't seem to think there is a form.

There is a form, but I have not reached that point yet. I filled out the form saying I am renewing and attached was the self health form (where they ask you health questions to answer). I am waiting for my CT to tell me when I have complete the medical exam. There is a date is must be completed by.

I'm with SMOE and no one has given me any forms about renewal or health checks.  Should I be concerned?

 Should the MOE automatically send the forms to my school... or does my co-teacher need to request them?

 I work in a small school and only have one co-teacher.  She's only been at this for three months and doesn't really know much about the system yet.  Maybe I need to be more proactive if I want to stick around another year... ??

Yes, you need to have your co-teacher contact your district. My friend was in your boat and her CT didn't know to even ask to her if she was renewing. She had to tell her and then the got the wheels rolling. They have to send the paperwork in and schedule your open class and evaluate you and sign you by mid-June I believe. SMOE requires all schools to confirm renewals by June 24th. I would definitely give your school a shove.

I usually get my health check form from the hospital that does the check for me. When the ball starts rolling depends not so much on SMOE as your district office and mine usually starts the first week of June, so if you have heard nothing by then get you co-teacher to ring your district office.

You pay for it yourself and the cheapest place is the hospital in Samsung, it is about 50,000 won, I know some charge a lot more, you must take two passport photos with you and they give you the form there at the hospital.

Does anyone know if we have to get our health check done by June or is that done again when our new contract starts?  I am traveling home in June for a family event from the first week mid and don't want to interfere so far my coteacher has been less then helpful.

It is not covered by your medical insurance. They do x-rays, drug and HIV tests, so I guess that is why it is expensive.

Nothing you can do about it, you just have to cough up and pay.

My school gave me and all the other teachers a form that was good for a free health check last year.  (I of course then had to lose it on my way to the hospital, and paid cash after all.)  If you're paying yourself, you might get a discount if your your employer is SMOE.  The hospital by the Coex gave me a SMOE discount last year, anyway. 

Remember to bring photos.  You need more of those slightly-smaller-than-passport-size photos. 

LittleBuddy: I was told I needed to have it done before signing my new contract.  Get it done before you go visit your family.  You can have the results sent to your school.

You do it once a year when you re-sign, so if you are re-signing for September then the process starts in June, so get it done then, so that when your contract is signed you can go to immigration and get all the paperwork done. If you start your contract in March then I think everything gets done towards the end of November.

Do you know the exact name of the hospital or the closest subway station? I'm reaaaaaaaally hoping to find a place which will do it for 50k!! ^^

You can go to Seoul Medical Center in Bonghwasan. Real close to the subway for 51,200.

What is the specific test called? Does anyone know the 한글 name for it as well?
This site lists all the national testing centers, I think. There seem to be several tests, all different prices. Expert opinions please!

It's called "겅강진다시". Go to Bonghwasan station and then go to Seoul Medical Center. As other posters have stated, the total cost is 51,200 WON. Go to the 2nd floor and there will be an English speaker who can help you.

Do you have to go for a health check when you transfer from one school to another after completing a contract?
I got my ARC extended, but no medical check-up. I've been with the new school a month now. I asked my co-teacher and she said she is not sure I should find out myself. :)
