EPIK medical for 2nd contract

Hi all,

I am changing provinces so I need to do all the paperwork etc. all over again.  I have to get a medical for my new visa/ARC.   Does anyone know if I need any forms etc for this and what exactly should I ask for when I go to the hospital?
