You can make the questions based off the topics you taught in your class. For example, if you taught a lesson on idioms, you could make a question about an idiom you discussed and practiced in the class. In my experience, they always ask for 3 multiple choice questions based on the content of my lessons. Make sure you choose lessons that all the classes have done. They also specified that there should be 5 choices for answers. I'll attach an example from a previous year's exam.
You should find out how many questions they want from you, and what style. Be aware that the students will complain that your questions are too difficult. In fact, they are not, but the problem is that they are not used to the question itself being in English, so they get confused as to what they are supposed to choose. Because of this, I always try to make the questions as simple as possible.
